Our History

2006
The Beginning
T&M Lifeline HomeCare Inc. was founded with a mission to provide compassionate care and support for individuals with intellectual and developmental disabilities (I/DD). Starting with a small adult foster home, our goal was to create a safe and nurturing environment where individuals could thrive.
2010
Expanding Our Care
With a growing reputation and an increasing demand for services, we expanded to multiple state-certified group homes across Portland, Oregon. This growth allowed us to support more individuals with I/DD and establish a foundation of quality care and trust in the community.
2015
Achieving Certification & Recognition
We became certified by the Oregon Department of Human Services (ODHS) and the Office of Developmental Disabilities Services (ODDS), validating our commitment to high-quality care and ensuring compliance with state and federal regulations. This milestone solidified our position as a trusted provider of residential care.
2018
Expanding Services with Life-Skills & Support Programs
We launched our Life-Skills Development and Behavioral Health Support programs to help individuals with I/DD gain independence and improve their quality of life. These services included personal care, social skills development, and emotional support, ensuring holistic care for our clients.
2022
Enhancing Behavioral Health Support
We expanded our services to specialize in dual diagnosis care, providing specialized support for individuals with mental health disorders alongside I/DD. This expansion allowed us to provide more comprehensive care for individuals experiencing complex behavioral challenges.
2025
Launch of Employment Agency
This year, we proudly launched our Employment Agency, focusing on connecting individuals with I/DD to meaningful job opportunities. Through job coaching, workplace support, and career development, we are helping individuals achieve greater independence and become active members of the workforce.

With nearly two decades of experience in residential and behavioral health services, he founded the organization to create a space where individuals with I/DD are valued, supported, and empowered. His leadership blends compassion with deep clinical and operational knowledge, ensuring every service provided meets the highest standard of care.
